Getting started with physical therapy can be a burden due to all the documentation your health insurance or doctor may require. So, what are the requirements to start physical therapy and get healing asap? Let’s dig in!
Your health insurance requirements may include:
- A script or referral from your doctor for physical therapy
- Authorization from your health insurance
- Insurance card information
- Or other various documentation specific to your insurance plan
Many health insurances require that you have all of this in place before attending your first visit.

What we’ll need
We’ll only need to ask you a few quick questions to get started – that’s it! We’ll need basic info like your name and birth date, and your health insurance information found on your card.
We’ve made it super convenient to answer these questions, either by visiting our physical therapy service page and filling out the quick form located at the bottom of the page, or alternatively, you can always call our clinic at 586-884-4565 and we’d be happy to help!
The Check in Express™ Process looks like this:
- Answer questions: Just answer a few basic questions, which you can complete online at this page or call us at 586-884-4565.
- Research: We’ll immediately use that info to look into any requirements your health insurance or doctor may require before starting therapy.
- Complete requirements: This is where our Check-In Express™ process shines! After identifying any requirements for you to start physical therapy, we make sure they’re taken care of. We can usually get all requirements within 24hrs or less.
- Contact: We’ll reach back out to you and review any requirements we’ve completed on your behalf with you and provide your health insurance benefits, so you know exactly what what to expect for your first visit. We can even schedule your first visit at this time if you’re ready to start therapy.
- Done! With everything out of the way in record time, you can start healing asap.

Will Your Insurance Cover Physical Therapy?
Typically, most insurance companies offer some level of coverage for physical therapy under each of their plans. That level of coverage can vary between each policy, however.
When it comes to plan and network types, you’re looking at the type of services that are covered in your plan. As well as the doctors you are allowed to see. These can either be in or out of network. Some plan types let you see any doctor. Other plans can limit your choices or charge you more to see out of network providers.
The different plan types include HMOs, PPOs, POSs, and EPOs. Each plan type has a different amount of freedom for choosing providers.
Depending on your plan, you may have to meet a deductible, pay a copay, or physical therapy may be covered in full at 100% by your insurance company.
Typically a policy will have a deductible, coinsurance or copayment, and an out of pocket maximum. Although every plan is different and may not have each of these, it’s important to understand them and how they work. It’s especially useful when it comes time to choose future health care plans.
Deductibles
A deductible is an amount you owe before your plan begins to pay.
The deductible may not apply to all health care services that you need. You may not need to pay for services like PT or chiropractic care. In these cases, the insurance company will pay some or all of the bill.
Coinsurance
A coinsurance is a percentage of the costs you are responsible for paying after you’ve met your deductible.
Coinsurance amounts can be different depending on the type of service. There may be 10% coinsurance for chiropractic visits and 20% coinsurance for exam visits. Some services may not require coinsurance at all.
Copayment
A copayment (or copay) is a dollar amount you pay for health care services. Copays are due after you’ve paid your deductible.
Most plans have some sort of copay for one or more services. Each service has a different copay. You may have a $30 copay for doctor visits, but a $50 copay for lab tests. Some services may not require a copay at all.
Out Of Pocket Max
The out-of-pocket maximum (OOPM) is the most you will ever have to pay during a policy period. Your deductible, coinsurance, and copays go toward your OOPM. Once you have met this amount, the insurance company will pay 100% of all remaining covered services for that year.
The OOPM does not include your monthly premiums. Think of your premium like paying for a gym membership. You pay $29.99 a month to use the gym. Your premium is the monthly payments you make each month to your insurance company to have healthcare coverage.
Know Your Coverage
As a courtesy, our Billing Department will verify your insurance benefits. If you do not have in network medical benefits, please speak with our billing coordinator to discuss self-pay options. There is no payment plan option for self-pay patients.
It is VERY important for each patient to know their individual benefits so unexpected balances do not occur. The Borja PT Billing Department files with many insurances that offer several different plans. Therefore, the patient must make sure their benefits are accurate.
Workers Compensation/Motor Vehicle Accidents
Additionally, we are happy to treat patients with personal injury or motor vehicle accident claims. The billing department will need certain information to verify each claim prior to service. For example, they will need the claim number, adjuster’s name, and contact phone number. As well as the claim mailing address.
Should the Workers Comp or Auto company deny your claims, the claims will then be submitted to your Medical Insurance. If you do not have medical insurance, however, the balance will become your responsibility. Please let us know if you have an attorney involved, along with their name and phone number as well.

How Many Physical Therapy Visits Are Needed?
Physical therapy focuses on joint alignment and movement patterns. They also look at imbalances and limitations in motion. They work to help restore movement. This involves treating the body as a whole. Typically, they will use stretches, exercises, and hands-on techniques. Their focus is to get people back to doing the things that they love.
The number of PT sessions you need will depend on a few things. For instance, the type of injury you have. As well as the severity of the injury. Other things, like your job, past medical history, and even your insurance can impact the number of visits needed.
During your first visit, the PT will start by asking questions about your injury or condition. They will ask how it happened, what movements cause pain, what activities you’re not able to do, and more. Next, they will ask about your medical history. They will also look at any imaging reports that you may have.
Then, they will take measurements, such as range of motion and muscle strength. They’ll do testing and get other information related to your injury. Based on this information, the therapist will create a Plan Of Care for you. They will also set goals to meet.
Follow-Up Sessions
Typically, injuries make the most progress from going to PT 3 times per week for the first 4-6 weeks. After that time period, the therapist will assess your condition and decide on the next phase in your Plan Of Care. Depending on what they see, they may decrease the frequency of your visits to 2 times per week.
Some Worker’s Comp insurers have a set amount of visits allowed for each condition. Additionally, some post-surgical conditions require PT for the first 5 days straight immediately after the surgery. Here at Borja PT, we believe that every person and every injury is unique. Therefore, we do not take a cookie-cutter approach when determining your Plan Of Care and your road to recovery.

How Long Is Each Session?
During your first visit, the therapist will start by asking questions about your injury or condition. They will ask how it happened, what movements cause pain, what activities you’re not able to do, and more. Next, they will ask about your past medical history. They will also look at any imaging reports that you may have. Then they will begin an assessment.
During the assessment, they will take measurements, such as range of motion and muscle strength. They will do testing and get other clinical information related to your injury. Based on this information, the therapist will create a specialized Plan Of Care for your recovery. They will also set goals to achieve throughout your Plan Of Care. The first visit will take around 45 minutes.
Follow-Up Sessions

Each follow-up session lasts around 45-60 minutes.
After your initial evaluation, the therapist will create a home exercise program for you. These exercises are designed to safely strengthen your muscles. All without the risk of making your injury worse. During the first few weeks, the therapist will advise you to only do the home exercise program. This will help the therapist track your condition and the effectiveness of the exercises. It also allows them to identify sources that may be causing pain or preventing your progression.
The primary goal of the home exercise program is to aid your recovery by strengthening specific muscles in a specific order. These exercises should be done on days when you have physical therapy appointments. More importantly, you should do them on the days that you do not have therapy.
Doing your exercises on the days that you don’t have therapy will help you recover quicker. Far beyond just physical therapy alone. They act as a one-two punch in your recovery, so to speak. In fact, you should do your home exercise program even after you’re done with physical therapy. It’s important to prioritize these exercises at least 3 times a week. This will prevent your injury or condition from coming back as a result. As you advance throughout your Plan Of Care, the therapist will add to your home exercise program.

Exercising with the gym equipment is part of your Plan Of Care. In fact, your therapist will incorporate it into your treatment. For instance, they will have you warm up on the exercise bikes. On the other hand, you may use the Life Fitness circuit machines to end each session. The equipment is used to improve strength, range of motion, and balance.
