When it comes to plan and network types, you’re looking at the type of health care services that are covered in your plan. As well as the doctors and facilities you are allowed to see. These can either be in network or out of network. Some plan types let you see any doctor at any health care facility. Other plans can limit your choices or charge you more to see out of network providers. 

The different plan types include HMOs, PPOs, POSs, and EPOs. Each plan type has a different amount of freedom for choosing healthcare providers.

Out Of Network

Health Maintenance Organizations (HMOs)

HMO’s are a type of health insurance plan that only allows you to see doctors within their network. This plan generally doesn’t cover out-of-network services, except in the case of an emergency. An HMO also requires you to choose a primary care physician to be eligible for coverage. The primary care physician helps coordinate your care. This means they must give you permission before seeing a specialist.

Preferred Provider Organizations (PPOs)

PPO’s are a type of health insurance plan that gives you better coverage for you to see the doctors within their network. You will pay less to see an in network doctor than you would an out of network doctor. You can still see doctors outside of that plan’s network, but you will have to pay more for those services.

Point Of Services (POSs)

POS’s are a type of health insurance plan where you pay less if you use doctors, hospitals, and other health care providers that belong to the plan’s network. Think of a POS plan as a hybrid between an HMO and PPO. You are choosing whether you want to use HMO or PPO services each time you go to the doctor. Like an HMO plan, you have to choose a primary care physician, but you can pay extra to see non-participating providers, as you would with a PPO plan. POS plans also require you to get a referral from your primary care physician if you need to see a specialist for any reason.

Exclusive Provider Organizations (EPOs)

EPO’s are a managed care plan where services are covered only if you go to doctors, specialists, or hospitals in the plan’s network, except in an emergency. Unlike an HMO, you don’t need to name a primary care physician or get a referral to see a specialist, as long as you choose a ‘participating provider.’

A network provider has agreed to provide services to the plan’s members at a discounted price.

Contact Our Billing Department

As a courtesy, our Billing Department will verify your insurance benefits. If you do not have in network medical benefits, please speak with our billing coordinator to discuss self-pay options. There is no payment plan option for self-pay patients.

It is VERY important for each patient to know their individual benefits so unexpected balances do not occur. The Borja PT Billing Department files with many insurances that offer several different plans. Therefore, the patient must make sure their benefits are accurate.

What Is A Coinsurance?

Coinsurance

A coinsurance is a percentage of the costs for health care services you are responsible for paying after you’ve met your deductible.

  • For example, if the health insurance plan’s allowed amount for an office visit is $100, and Dwayne has met his deductible already, his 20% coinsurance payment would end up being $20 (20% of $100).
  • The health insurance company will then pay the rest of the allowed amount ($80).

Coinsurance amounts can be different depending on the type of service. You may have a 10% coinsurance for chiropractic visits yet have a 20% coinsurance for exam visits. Some services may not require a coinsurance at all.

deductible is an amount you owe for health care services before your health insurance plan begins to pay. 

  • For example, if Dwayne Johnson’s deductible is $2,000, he must pay the first $2000 himself.
  • After he pays his deductible, he might have to pay a coinsurance or copayment. The insurance company pays the rest of the bill.
  • In some instances, there is no coinsurance or copayment. Once the deductible is met, the insurance company pays the entire bill.

The deductible may not apply to all health care services that you need. You may not need to pay for services like physical therapy or chiropractic care. In these cases, the insurance company will pay some or all of the bill.

Family Deductibles

Family plans often have both an individual deductible and a family deductible. The individual deductible applies to each person. It’s a smaller amount that each person pays. The family deductible applies to all family members as a whole. This is a larger amount that each member collectively pays.

What Is A Copayment?

A copayment (also referred to as a copay) is a nice, round dollar amount you pay for health care services after you’ve paid your deductible.

  • For example, Dwayne’s health insurance plan’s cost for a doctor’s visit is $100. His particular plan has a copayment charge of $15 for office visits.
  • If Dwayne has already met his deductible, then he will only be responsible for paying $15 at the time of service.
  • If he hasn’t met his deductible, however, he will pay $100, the full allowable amount for the visit.

For the most part, all insurance plans have some sort of copay for one service or another. Copayment amounts can vary for different services. You may have a $30 copay for specialist visits yet have a $50 copay for lab tests. Some services may not require a copay at all.

We encourage our patients to wear looser clothing, such as workout clothes, to their therapy sessions. This allows the therapist to easily access your injury. It also doesn’t restrict your movements during your sessions.

Changing Room

We offer private, secure areas to keep your belongings next to each treatment table. We recommend arriving a few minutes before your scheduled appointment so you have time to change. This helps to make sure you spend your time working with the physical therapist.

Here at Borja PT, we have office policies in place to promote the safety of our patients, as well as to make ensure that each patient receives the proper care that they rightfully deserve.

We realize that emergencies and other scheduling conflicts come up and are sometimes unavoidable, however, advance notification allows us to fulfill other clients scheduling needs and keeps the clinic operating at its most efficient level. We strive to promote a higher quality of care at Borja PT, and as such, missed, or late appointments are a disruption to the clinic, your physical therapist and other patients.

Office Policies

Cancellation Policy.

If you ever need to cancel your appointment with us, you may contact the clinic at (586) 884-4565 to leave a message, or send an email to [email protected]. Additionally, you can text our staff at (586) 884-4565. Please provide your name, phone number, appointment date & time, and reason for the cancellation.

On Monday morning, you will receive a call to confirm that your appointment has been canceled. In addition, you will be able to reschedule your appointment at this time as well. We require 24 hours’ notice for the cancellation of an appointment in order to avoid a $15.00 cancellation fee. Click here to read more about our cancellation policy.

Do Not Show Policy.

We kindly ask that you provide our office with 24-hour notice to change or cancel an appointment. Patients who do not attend a scheduled appointment may be responsible for a $25.00 fee. You cannot bill this charge to insurance. Additionally, you must pay your fee on or before the next scheduled appointment. 

Accident claims adjusters expect regular attendance to physical therapy. In fact, it’s a requirement of the approved treatment plan. If appointments are missed or canceled on a regular basis, it could affect the status of your claim. Multiple missed visits may result in discharge from care. Same-Day Schedule Policy.

Our same-day schedule policy is for patients who have missed two or more appointments without providing notice. This means you would not schedule your appointments in advance. Instead, you would simply call, text, or email our office on the day you would like to attend therapy to see what times are available. Our front desk staff will then reserve the time slot of your choice for that day.

Missing your appointments can disrupt the clinic and your physical therapist. In addition, it can even disrupt other patients as well. While sometimes you may be running behind, or forget about your appointment altogether, it’s important to respect your therapist’s time. Financial Policy.

As a courtesy, the Billing Department verifies your insurance benefitsPlease provide your insurance cards prior to your first visit. There are 3 different ways you can pay for your visits.

Option #1

First, if you would like to wait until your insurance company determines your patient responsibility for each visit, we are happy to do so. In order to do this, you must place a credit card on file for automated payment at the end of each month.

Option #2

You may also enroll in a payment plan. This automatically charges 10% of your estimated total costs at the beginning of each week until the total balance for your entire plan of care is paid in full. Payment plans require credit card or bank account information to be put on file for direct withdrawals.

Option #3

Lastly, if you would prefer to not put a credit card on file, you can choose to pay each visit. We require an estimated payment at the time of service.

If you are a former patient and wish to return to physical therapy while there is still an outstanding balance on file, you must pay off the remaining balance in full. You may also enter into a payment plan agreement as well. As previously stated, the Borja PT Billing Department may work with you to create a payment plan based on the remaining balance in question. Type Of Equipment

After your evaluation, the physical therapist will create a home exercise program for you. These exercises safely strengthen your muscles. All without the risk of making your injury worse. Then the therapist will add to your home exercise program as you progress.

Using The Equipment

Exercising with the gym equipment is part of your Plan Of Care. In fact, your therapist will incorporate it into your treatment. For instance, they will have you warm up on the exercise bikes. On the other hand, you may use the Life Fitness circuit machines to end each session. The equipment is used to improve strength, range of motion, and balance.

The clinic is on ground level. It is accessible for wheelchairs, walkers, crutches, scooters, and any other transportation devices. The sidewalk ramp is located in the middle of the complex, about 15 feet from our front door. In addition, there is handicap parking available for easy ramp access as well.

During your first visit, the therapist will start by asking questions about your injury or condition. They will ask how it happened, what movements cause pain, what activities you’re not able to do, and more. Next, they will ask about your past medical history. They will also look at any imaging that you may have. Then they will begin an assessment.

X-Rays And MRI Results

We encourage patients to bring any imaging that they may have to their first visit. These reports can help give further details regarding your injury or condition. They may help the therapist when figuring out your course of treatment.

Some injuries or conditions may not require imaging, however. So don’t worry if you do not have any imaging or imaging reports. The therapist performs specialized testing during your first visit that provides details beyond information found in imaging.

The OOPM does not include your monthly premiums. Think of your premium like paying for a gym membership. You pay $29.99 a month to use the gym. Your premium is the monthly payments you make each month to your insurance company to have healthcare coverage.

When it comes to plan and network types, you’re looking at the services that are covered in your plan. As well as the doctors you are allowed to see. These can either be in or out of network. For instance, some plan types let you see any doctor at any health care facility. While other plans can limit your choices. Some may even charge you more to see out of network providers as a result.

If your insurance isn’t listed, however, please call our office to speak with our team. As a courtesy, the Billing Department verifies your insurance benefits. So, it’s important to provide your insurance information before your first visit.

They will be able to look into your primary health insurance policy. As well as any auto insurance policy, Workers Comp claim, and any secondary health insurance policies you may have.

Workers Compensation/Motor Vehicle Accidents

Additionally, we are happy to treat patients with personal injury or motor vehicle accident claims. The billing department will need certain information to verify each claim prior to service. For example, they will need the claim number, adjuster’s name, and contact phone number. As well as the claim mailing address. 

Should the Workers Comp or Auto company deny your claims, the claims will then be submitted to your Medical Insurance. If you do not have medical insurance, however, the balance will become your responsibility.  Please let us know if you have an attorney involved, along with their name and phone number as well.

During your first visit, the therapist will start by asking questions about your injury or condition. They will ask how it happened, what movements cause pain, what activities you’re not able to do, and more. Next, they will ask about your past medical history. They will also look at any imaging reports that you may have, as well as a list of your medications. Then they will begin an assessment.

List Of Medications

We encourage patients to bring a list of their current medications to their first visit. It is very important to have an up-to-date list of medications on file. As with any medication, there may be limitations or precautions that should be known. They may help the therapist when figuring out your course of treatment.
